Endoscopic Sinus Surgery for Chronic Paranasal Sinusitis in Aspirin Sensitive Asthma Patients

Abstract
There has been controversy in the efficacy of the surgical management of sinusitis for bronchial asthma. Aspirin sensitivity was confirmed by oral challenge test with 180mg of lysine-aspirin. To evaluate the result of the management of sinusitis
on
bronchial asthma, conservative management with combination of vigorous dressing follow-up, topical nasal steroids, antihistamines and appropriate oral antibiotics was done to 19 aspirin sensitive asthma patients with chronic sinusitis confirmed
by
computed tomography of ostio-meatal unit(OMU-CT). Six out of 19 patients(32%) showed improvement by intense medical management. Endoscopic sinue operations were done for 13 patients irresponsive to medical treatment.
After surgery, eight out of 13 patients(60%) showed improvement in nasal symptoms, and nine out of 13 patients(69%) in lower respiratory symptoms by symptom score. In 10 patients(77%), medication(theophylline, oral steroid etc.) requirement for
asthma
has been markedly decreased. Average follow0up period was 7.8 months.
